As of October 1, 2010, the Medicare/Medicaid Regulations are in effect regarding Minimum Data Set (MDS) 3.0, which includes an enhanced Section Q – Community Living. As part of the requirements for implementing MDS 3.0 Section Q, Virginia has established a statewide network of Local Contact Agencies (LCAs) to serve as the initial point of contact for individuals who are interested in receiving information about returning to the community. DMAS has designated the Area Agencies on Aging (AAAs) in Virginia, to serve as LCAs, providing introductory community resource information for individuals interested in transitioning back to the community.
The following spreadsheet lists the Local Contact Agency (LCA) and staff information for every city/county in Virginia. It is presented in alpha order by locality; however, because it is in an Excel spreadsheet, it can be downloaded and easily sorted by agency. In some cases, more than one contact within an LCA may be listed for a locality. Unless otherwise noted as “back-up”, there is no priority to the staff listings within a locality.

Information is updated weekly to provide current contact information to nursing facilities and other appropriate agencies and entities who are directly involved in implementation of MDS 3.0, Section Q. LCAs should immediately submit updates to Cecily Slasor whenever a change in LCA staff information occurs.
